With incredibly heavy hearts, we share the news that all KindlyMD clinics will close this summer. Our final day for behavioral health services will be May 22, 2026, with all other services ending on June 18, 2026, when we close our doors for good.

To ensure continuity of care, we strongly encourage you to find another provider as soon as possible. Please note that provider coverage may be subject to your individual insurance plan. Your insurance company may also provide a directory to assist you. Download a list of providers that may be accepting new patients here.

If you are a Medical Cannabis Cardholder, your card remains active until the expiration date. When you need to renew, you can find a new Recommending Medical Provider (“RMP”) on the Utah DHHS RMP directory or refer to your local dispensary for assistance.

KindlyMD will arrange for the secure storage of all patient medical records. Information regarding long-term records storage and how to obtain a copy of your individual medical records in the future will be available on our website in the coming weeks.

Currently, if you would like a copy of your medical records or wish to have them transferred to another provider before we close, please obtain a health records request form here or send an email to [email protected]. You may also access your records through the KindlyMD Patient Portal.
